Ozone Machine Rental Agreement & Safety Checklist

This Ozone Machine Rental Agreement (“Agreement”) is entered into between D & D Pest Control Co. (“Company”) and the customer renting the ozone generator (“Renter”).

By renting and operating an ozone machine from D & D Pest Control Co., the Renter acknowledges that ozone shock treatment is intended for temporary, unoccupied use only and agrees to comply with all safety requirements outlined below.


1. Purpose of Rental

The ozone machine is rented solely for the purpose of performing a short-term ozone shock treatment to help reduce stubborn odors, including but not limited to smoke odors, pet odors, musty odors, and post-removal dead animal odors.

The ozone machine is not an air purifier, is not intended for continuous use, and must never be operated in an occupied space.


2. Unoccupied Space Requirement (Mandatory)

The Renter agrees that the ozone machine will only be operated in a space that is completely unoccupied.

  • No people may remain inside during operation
  • No pets may remain inside during operation
  • No plants may remain inside during operation

The Renter agrees to secure all entrances and post warnings to prevent accidental entry while the ozone machine is operating.


3. Health & Safety Disclosure

Ozone at elevated concentrations can irritate the lungs and respiratory system. Federal and state health agencies warn that indoor ozone levels produced by ozone generators may exceed health-based standards if not properly controlled.

The Renter acknowledges that:

  • Ozone exposure can cause coughing, chest discomfort, and breathing irritation
  • Improper use may create unsafe indoor air conditions
  • Strict adherence to run time and ventilation procedures is required

4. Operation Rules

  • Operate the ozone machine only for the recommended time based on room size
  • Do not exceed suggested run times
  • Do not operate near flammable vapors or open flames
  • Do not tamper with machine settings or safety features
  • Remove or protect sensitive items (natural rubber, delicate plastics, artwork)

D & D Pest Control Co. is not responsible for damage to materials caused by ozone exposure when instructions are not followed.


5. Post-Treatment Ventilation (Required)

After the ozone machine has shut off, the Renter agrees to:

  • Allow the recommended dwell time to pass
  • Open windows and doors to ventilate the space
  • Use fans if needed to exchange indoor air
  • Ensure ozone odor has dissipated before re-entry

No one may re-enter the treated space until adequate ventilation has been completed.


6. Limitations of Ozone Treatment

The Renter understands that ozone shock treatment:

  • Does not remove stains, residues, or damaged materials
  • Does not replace cleaning, repairs, or moisture control
  • Is most effective when odor sources have already been removed
  • Is not a substitute for mold remediation or pest control

7. Liability & Indemnification

The Renter assumes all responsibility for the proper use of the ozone machine. D & D Pest Control Co. shall not be held liable for:

  • Injury or health effects resulting from improper use
  • Failure to follow unoccupied-space requirements
  • Insufficient ventilation after treatment
  • Damage to contents or materials due to ozone exposure

The Renter agrees to indemnify and hold harmless D & D Pest Control Co. from all claims arising from misuse or failure to follow this Agreement.


8. Acknowledgment & Acceptance

By renting the ozone machine, the Renter confirms that they have read, understand, and agree to all terms of this Ozone Machine Rental Agreement and Safety Checklist.

If the Renter is unsure whether ozone shock treatment is appropriate for their situation, they agree to consult with D & D Pest Control Co. before operating the equipment.
